Instructions: Fill in date ranges at start of semester. At the end of each week of work, student should rate themselves, have a mentor (usually a more senior student on the same project) rate their work (& give feedback) and sign their initials, then pass the sheet to the instructor, who may also input an opinion & give feedback. These 3 grades will be averaged together to produce a grade for the week. The average of all weeks so far should be written in the rightmost column. The semester grade will be the average of all 15 weeks' work, rounded to the nearest half GPA point.
